A Letter To Our Patients

By now, you will know that big changes are on the way for our office! We are moving to a nearby location, just 2 blocks from our former office!

Here are some details of our transition

Due to the transition, we were unable to have our appointments transfer to the new office after the first of the year. If you have an upcoming appointment that was scheduled at our previous office, please call the new office at (615)986-6039 to confirm. 

You don't have to switch hospitals! We will continue to practice at both Saint Thomas Midtown Hospital and Centennial Women's Hospital in Nashville.

We will continue to take all of the same insurance plans as before.

We have decided to end our business relationship with the corporation Tennessee Women's Care.

It is NOT necessary to transfer your chart to continue seeing Dr. Reynolds, Dr. Maikis, Dr. Binkley, or Dr. Thornburg for your care.

However, since Tennessee Women's Care owns your patient records, it is your choice to execute a medical records release to have your records transferred to our new office.  Please note that there will be a small fee associated with the transfer.

If you do wish to have your chart transferred please download and print our release form by clicking here.  You may bring the form to the office at the time of your next appointment, fax it to our new office at (615)234-1520, or mail it to our new address 330 23rd Ave N, Suite 604, Nashville, TN 37203.

If you chose to have your records transferred through our old office prior to our move, please notify our new office at (615)986-6039 so that we can make sure they arrive.
